Virtual Patient Grtoup

Our virtual patient group is now live. If you want more information about it and to register to join please visit the Patient Group tab on the right hand side- select the virtual patient group tab, complete the form and return to us.

If you would like to join the patient group meetings which take place in the surgery please leave your name with reception. The next meeting will be held 29th September at 1 pm, new members welcome.

Accessible Information Standard

The Accessible Information Standard aims to ensure that patients (or their carers) who have a disability or sensory loss receive information they can access and understand. For example in large print, braille or via email.

As part of the Accessible Information Standard we must:

  • Identify patients who may have an additional communication need.
  • Record the patient's need in our clinical GP system.
  • Flag the patient's record to assure additional communication needs are obvious to practice staff;
  • Share this information with other care givers such as social care;
  • Act to ensure our staff can effectively communicate with patients who may have additional communication needs.

Please inform a member of staff in reception if you have any communication support needs- you can call us on 0207 249 5511 or tell a receptionist in person. You need to help us make sure we get things right for you.

Baby Clinic Changes

We are changing immunisations to APPOINTMENT ONLY in Baby Clinic

Please note that from Wednesday 6th April 2016, Nurse’s baby clinic arrangements will change. Nurse appointments for immunisations will be by appointment only.  Our baby clinics are on Wednesday Afternoon 1:30 to 3:30pm and Thursday Morning 9:30 to 11:30am.  Please also note that you could see a health visitor on Wednesdays only. On Thursdays health visitors can make contact via telephone if advice is required.

Doctor's baby clinic, the first hour is by appointment only for six week checks and the second hour is walk-in for other patients who want to see a Doctor.

Named Accountable GP for all Patients

From 1st April 2015, all practices are required to inform their patients including children of their named accountable GP, under the new GP contract for 2015/2016. The named GP will have overall responsibility for the care and support that the surgery provides to you.

If you would like to know who your named GP is please contact reception.

This does not prevent you from seeing any GP in the practice as you currently do.

Urgent on the day Appointments

Please ring the surgery in the morning from 8.30am to get a timed appointment on weekdays.

Repeat Prescription

Please allow 48 hours for processing of your repeat prescription once you have dropped it off at the surgery. We do not take requests for repeat prescriptions over the telephone.

Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors or nurses, please telephone to cancel your appointment.

Out of Hours

Call CHUHSE on 0208 185 0545. You will be connected to CHUHSE our out-of-hours service provider. Telephone calls to the out of hours service are routinely tape-recorded as a safe-guard for patients and staff alike

REPEAT PRESCRIPTIONS

To request a repeat prescription online, you will first need to register with Patient Access. You will need to come to the surgery in person with a photo ID (please see below for acceptable list of photo IDs) so we can provide you with a PIN which you will need before you can register with this service, once you have the PIN, please click here to register with Patient Access. You will be able to order your repeat prescription directly through this site once registered.

If you need to order an acute medication you can either bring your request to the reception desk during surgery times, or post to the surgery with a stamped addressed envelope or send a request by fax or (cahccg.prescriptions.bartonhouse@nhs.net) email us here.

Please allow 48 hours for your prescription to be ready for collection .

Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.

Please do not ask reception staff to prepare the prescription for you on the spot. The doctors need to check your record before signing the prescription.

Please note, the contraceptive pill cannot be processed as an ordinary repeat medication. You must make an appointment to see the nurse before a prescription can be issued. Please ensure that you leave sufficient time before you run out of your pill to make an appointment to see the nurse.

Acceptable list of photo IDs are:

Passports, UK/EEA/EU photo driving licence, Student ID cards, Senior citizen bus passes, EU/EEA national identity card,  Armed forces ID card.
