Sharing Your Medical Record

In order to provide a good standard of care good communication between different NHS services is often required. If you consent to this some of your health information will be shared and viewed by other relevant local NHS services. For example a local community team might be able to see your current medication list from your GP records.

The NHS Summary Care Record (SCR) is an electronic summary of key clinical information (including medicines, allergies and adverse reactions) about a patient, sourced from the GP record. It is used by authorised healthcare professionals to support their care and treatment.
